The forms below are used to get to know you as a client.  You may fill out the Client Health Intake Form before your massage session so that you can focus on relaxing upon your arrival at my office.

 We will discuss the specific outcomes you’d like to accomplish during our time together and how massage can help to achieve those goals.

If there is any question as to whether you should receive  massage therapy, please discuss the matter with your primary doctor.  Print out the Physician's Permission form and have your doctor sign it before your massage session.

Your doctor may recommend massage therapy for you- this is great!  Below you will find the Physician's referral form.  This may be used if your insurance requires it to pay for massage services. 

At this time I do not accept insurance.  Contact your insurance provider to find out what guidelines need to be followed if you have benefits that cover massage therapy services.  I am able to provide receipts for reimbursement, however, I cannot accept insurance to pay for massage therapy services at this time.
